2025 Form 1095-C Document Download Reminder
The 2025 form is now available for download.
All documents will appear on the member’s record as they generate under personal documents in the BenefitSolver portal.
To access your form:
- Go to www.88Sears.com
- Under Quick Links, click on Health Benefit Center
- Log in using your Enterprise ID and Password
- Open Personal Documents and Download your Form 1095-C
Associates/former associates can obtain a paper copy of the form upon request only by sending an email to BenefitsDepartment@Transformco.com.
State Mandates Still Apply
Some states (e.g., California, New Jersey, Rhode Island, and Washington, D.C.) still require paper distribution of Form 1095 regardless of federal changes. Transformco will comply with these state-specific mandates and provide all associates/former associates a paper copy to the home address on file.
The forms are no longer automatically printed and mailed unless you live in one of the mandated states that require paper distribution of Form 1095 regardless of the federal changes, these include (California, New Jersey, Rhode Island and Washington D.C.).
Important Note
In accordance with the Paperwork Reduction Act (PRA), paper copies of the form will be mailed upon request only. The PRA is a federal law designed to reduce unnecessary paperwork and improve efficiency by encouraging electronic access to documents while still ensuring paper copies are available to those who need them.
